Filling out this registration will enroll you in Sharing the Stories. We want to be part of your journey and help you tell your story confidentially. We will provide you feedback right away and for as long as you keep logging back in. You can come back whenever you want to see results and add more information.
We’ll ask you some questions about yourself first to set up your account. Then we’ll tell you more details about consent to the research we’ll be doing. We’ll explain how we will use and protect your information, and how we will keep your name and identity totally separate from any information you provide. In order to proceed, you will be asked to click a consent button. If you don’t click the consent button, all the information you provided about you, will not be saved and will not have been stored anywhere.

Your account is the first step in the registration process to become a part of the SCC network which provides you with access to event, program and research opportunities (through our Sharing the Stories (StS) platform). We’ll ask you some questions about yourself first to set up your account.
We work hard to protect your personal information; we do not share or sell it to anyone; it’s stored on encrypted servers in Canada. We use it to provide you with services like conferences, and we use anonymized data (no names or people connected to it) for our planning and evaluation.
We ask that you provide your birthdate so that we can ensure that young people's information is especially protected. All other fields in the “About You” section are optional when creating an account with us, but any information you do choose to share supports us in ensuring our programs and events meet your needs. By filling out information in the “About You” section, you consent to the SCC to store that information for program and event purposes.
Through STS you can log back in and stay connected, see results, and apply for new things.
The SCC leads a research study on youth programs and what makes them work well or not. You can help us with this study and give feedback on every program you participate in. You can see the results through your STS log-in. This letter provides more information. You do not have to participate in the research study in order to participate in SCC programs and events.
We are asking participants in this program to take part in a research study examining how youth and young adults engage in this program. If you agree to participate, your survey results will be added to the survey results of youth and young adults across the country. There are no known risks for taking part in this study.
There is no obligation for you to say yes to take part in this study. You don’t have to answer any questions you don’t want to. You can stop participating at any time without penalty. You may withdraw from the study for three months after completing the study by contacting Christa Romaldi at email@example.com. There are no direct benefits to you for participating in this research.
We will keep your data securely indefinitely. Your confidentiality will be protected to the extent possible by replacing your name with a unique code for all data and in all publications. The code list linking real names with unique codes will be stored separately and securely from the data. Other than the research team, only research assistants who have signed a Confidentiality Agreement will have access to any of the data.
We hope to publish the results of this study in academic journals and present them at conferences. All information will be presented at the group level. There will be no way to trace your responses directly to you.

This Letter of Information provides you with the details to help you make an informed choice. All your questions should be answered to your satisfaction before you decide whether or not to participate in this research study.
